Rates:
- First appointment (up to 90 minutes): $180
- Subsequent appointments (60 minutes): $150
- I am in-network with the following health insurance plans:
- Premera BlueCross
- Regence BlueShield
- Kaiser Permanente Core and Options plans
- If you have health insurance benefits with another company, contact that company to find out if you have out-of-network benefits. You will need to pay the full fee at the time of service and submit claims to your insurance for reimbursement.
- Payment of the full fee (or patient cost-share, if I’m an in-network provider for you) is expected at the time of service.
- Forms of payment accepted: cash, check, and major debit/credit cards.
Location:
- Broadway Commercial Center — 2209 Elm St., Suite 203 — corner of Elm St. and Broadway St.
- Located just north of downtown Bellingham, between the Columbia and Lettered Streets neighborhoods
- Free parking is available in a carport at north end of building, or in the alley behind the building
- To find my office, take the stairs next to the carport (on Elm St.) to the 2nd floor. Enter the first door on the right & be seated in the waiting room.
